Personalized Electronic Medical Record (PEMR)

PotentiaMED’s personalized electronic medical record (EMR) tool that allows patients and providers to create a record following a triggering event, such as a specific diagnosis, the scheduling of a procedure, or the emergence of a particular condition.

Upon patient consent, the physician’s medical staff directs PotentiaMED to extract the relevant information from PMED intelligence to automatically populate the record. PMED PEMR has the capability to accept data from remote devices and accelerates the exchange of critical consumer health information such as prescription drug information, lab results, and medical imaging to creating a more dynamic and responsive environment for the patient, physician and hospital through increased information liquidity. Using our propriety process and data sources, the record is accessible to the patient and caregivers on the “mypemr.com” website or smart phone, and serves as a unique link between patients, caregivers, providers and life sciences firms.
